If you are injured at work, or you see something unsafe (a near-miss), tell the Duty Supervisor straight away.
Use the Incident and Near-Miss Report Form to record what happened as soon as possible. This helps the centre fix hazards and meet legal workplace safety requirements.
Include:
- your name and role
- date and time
- location
- what happened (facts only)
- any immediate actions taken
Submit the completed form to the Duty Supervisor before the end of your shift. If it is a serious injury, call 000 first.
List one clue from the text that shows who the intended audience is.
